Evaluating the effect of chronic sleep restriction before myocardial ischemia on cardiac function and structure in male rats

Introduction: Sleep restriction (SR) can have significant adverse cardiovascular consequences. Untreated SR may lead to poor functional outcomes following ischemia and increase the risk for myocardial ischemia (MI) morbidity. Here we investigated the influence of chronic SR before MI on cardiac function and structure in rat models.
Methods and Materials: Male Wistar rats (n=24) were randomly assigned to 4 experimental groups: 1) Control; 2) Sleep restriction (SR); 3) Myocardial ischemia (MI); 4) Sleep restriction and myocardial ischemia (SR+MI). SR and SR+MI animals were subjected to a chronic SR paradigm for 14 days. To induce MI, isoproterenol (130mg/kg/day) injected subcutaneously on two consecutive days. Blood sampling was done 1 day after isoproterenol injection for evaluation of the plasma level of Lactate Dehydrogenase (LDH). Echocardiography was used to define cardiac structure and function 28 days after isoproterenol injection. Masson’s trichrome staining and hematoxylin and eosin (H&E) were used to detect fibrosis and myocardial histology respectively.
Results: In the current study SR before ischemia attenuates functional parameters such as ejection fraction (%EF) and fractional shortening (%FS) in comparison to the MI group (p<0.05). Serum levels of LDH significantly increased in MI and SR+MI groups in comparison to control. Destruction changes of left ventricular tissue and cardiac fibrosis in SR and SR+MI rats increased considerably compared to the control group. All of the mentioned parameters had higher levels in animals that had SR induction before MI (p<0.05).
Conclusions: Our data suggests that the experience of chronic sleep disruption before MI leads to unfavorable left ventricular remodeling and exacerbates poor functional and structural outcomes following ischemia.
