مقایسه تاثیر دو روش گرم کردن بیماران تحت عمل جراحی لاپاراسکوپی بر شاخص‌های فیزیولوژیک

هدف: هیپوترمی از عوارض شایع و جدی می‌باشد که در بیماران تحت جراحی لاپاراسکوپی رخ می‌دهد. روش‌های گرم‌سازی متنوعی برای پیشگیری از این شرایط در اتاق عمل مورد استفاده قرار می‌گیرد. هدف از این مطالعه مقایسه تاثیر دو روش گرم کردن بیماران تحت جراحی لاپاراسکوپی بر شاخص‌های فیزیولوژیک بیماران می‌باشد. مواد و روش‌ها: در یک مطالعه کارآزمایی بالینی 96 بیمار تحت جراحی کله سیستکتومی لاپاراسکوپیک، در بیمارستان الزهرا (س) شهر اصفهان به‌صورت تصادفی در سه گروه (سیستم گرمایشی با فشار هوا، سرم گرم وریدی و کنترل) قرار گرفتند (هر گروه 32 نفر). مداخله بلافاصله پس از القای بی‌هوشی صورت گرفت. دمای مرکزی بدن، SPO2 و تعداد تنفس، هر 15 دقیقه تا 30 دقیقه پس از اتمام جراحی و شدت لرز نیز با ابـزار مـشاهده‌ای Grossely و Mahajan در ریکاوری ثبت شد. یافته‌ها: میانگین دمای مرکزی بدن، تعداد تنفس و SPO2 در هیچ یک از زمان‌ها (قبل از القای بی‌هوشی، حین عمل و پس از عمل) بین سه گروه اختلاف معنادار نداشت (05/0P>) و لرز پس از عمل در هیچ یک از سه گروه مشاهده نشد. نتیجه‌گیری: استفاده از روش‌های فعال گرم‌سازی از جمله سرم گرم وریدی و سیستم گرمایشی با فشار هوا در جراحی‌های لاپاراسکوپی، تاثیری بر شاخص‌های فیزیولوژیک نداشته است. لذا توصیه می‌شـود بـا توجـه به سـایر شـرایط بیمـاران، روش گرم‌سازی انتخـاب گردد.
